This in-depth presentation looks at Froedtert's Adult Vaccination Uptake Improvement Program, an initiative launched in May 2024 to educate and inform unvaccinated patients about the benefits of flu and COVID vaccines. Through a collaborative effort across multiple teams—Pharmacy, Population Health, and Data Analytics—this program has successfully expanded from two pilot clinics to seven locations, improving patient access to vital vaccinations. Discover how the team has developed a referral process, integrated data tracking systems, and equipped healthcare providers with motivational interviewing training to address vaccine hesitancy.
